Thank you for being a Playlist for Life Help Point. We hope our materials and training have been useful to you over the past few months. Please take a few moments to fill in the questions below. You don't have to answer every question but any feedback or suggestions you can give us will help us to improve our services.

How many people with dementia or carers have you reached with our playlist resources, i.e. our getting-started leaflets or conversation starters? (A guesstimate is fine!)
