Playlist Request Service: make a referral

This form allows you to refer someone to our remote Playlist Request Service. A volunteer Music Detective will use the information you provide to make a playlist for the person you refer. Please complete the Music Detective details as fully as possible. The more information you provide, the more useful the playlist will be.

If you do this then you must obtain their consent first so that they are aware we will be sending them a playlist and storing some of their personal information in our systems. Please tell us any information about the musical likes of the person you are referring.

Do they have any favourite singers or bands? Do they like a particular genre of music, e.g. jazz or classical or country & wester? Do they play a musical instrument? Are they a member of a choir? Or were they in the past?

Perhaps tell us a little bit about their life history. For example, people who are church members often find hymns important. People who used to be members of the Guides or Scouts or other organisations will often remember the music associated with those groups. Or maybe the person spent some of their life living in another region or another country and will have happy memories associated with the music of that place.

And, if there are any songs, bands, singers or genres that the person actively dislikes please tell us too so we known not to include any of those!
